Welcome to Madame Driving School Corp. These Terms and Conditions outline the policies
governing all services, lessons, and appointments with our school. By enrolling in or scheduling
any service with Madame Driving School Corp., you agree to abide by these terms.

1. Lesson and Appointment Policies

1.1. Cancellation Policy:
• A minimum of 24 hours’ notice is required for all lesson cancellations.

• Cancellations made with less than 24 hours’ notice will incur a $29 no-show fee per scheduled class.

• No cancellations are permitted on the same day a class is scheduled.

1.2. Punctuality:
• Students are required to be on time for all scheduled lessons.

• Lost time due to student lateness will not be reimbursed.

• If a student is more than 10 minutes late for a lesson, the lesson will be forfeited, and no
refund or rescheduling will be provided for that missed time.

1.3. School Rescheduling:
• Should Madame Driving School Corp. need to reschedule a class or adjust a lesson time,
students will be notified within 24 hours of the original class time.

• If Madame Driving School Corp. is late for any reason, the student will receive their full
45 minutes of instruction.

2. Road Test Policies

2.1. Road Test No-Show and Rescheduling Fees:

• A $59 fee will be charged for a “no-show” at a scheduled Road Test.

• All Road Tests must be canceled at least 24 hours prior to the exam date to avoid a fee.

• A $19.00 fee will be applied for rescheduling Road Test appointments.

2.2. Road Test Transportation:

• Madame Driving School Corp. does not provide pick-up services from a student’s home
or workplace for road tests. Students are required to meet their instructor at the school’s
office, located at 3340 Fort Independence Street, Bronx, NY 10463.

2.3. Road Test Preparation & Required Documents:
• Students must arrive at the school’s office at least one hour before their scheduled road
test to prepare and ensure all documentation is in order.

• The following items are mandatory for the Road Test:

o Your valid learner’s permit.

o Glasses or hearing aid (if required by your permit).

o If the 5-hour course was completed with another driving school, the official 5-hour certificate must be presented.

o If the 5-hour pre-license course was completed with Madame Driving School
Corp., your certificate will be ready for you.
o If the 5-hour course was completed online with a DMV-approved company, the
DMV will automatically have your certificate on file.

• For students aged 16-18, the MV-262 form must be signed by a parent/guardian (18
years of age or older). Failure to provide this signed form will prevent the student from
taking the Road Test.

• If your permit has a “B restriction,” the Department of Motor Vehicles (DMV) requires
you to have your glasses or contact case present at the time of the Road Test.

• Temporary visitor documents must not be expired at the time of the Road Test. An
expired document will result in the inability to take the exam.

• Failure to bring any of the required items will result in the student not being able to
take their road test and will incur a charge to reschedule.

2.4. Road Test Location Fees:
• An additional $25 fee applies for Road Tests conducted in White Plains, Yonkers, or
New Rochelle.

• Road Tests conducted in the Bronx are included in our packages with no additional fee.

3. Required Documents for Lessons and Classes
• Students must bring their valid learner’s permit to every scheduled lesson, 5-hour
class, and Road Test.

• Failure to have your permit for a lesson will result in a $29 no-show fee.

• You will not be permitted to take your 5-hour class or Road Test without your permit.

4. Packages and Refunds

4.1. Package Validity:
• All purchased packages are valid for one year from the date of purchase, with no
exceptions.

4.2. Refund Policy:

• Except for contracts executed by schools licensed by the New York State Education
Department and subject to their specific refund provisions, prepayment for lessons and
other services shall be subject to refund as follows:

• If the student provides prior notice of at least 24 hours and withdraws from or
discontinues a prepaid course of instruction or series of lessons before completion, or
from any other prepaid service, or if the school is unable or unwilling to complete such
prepaid service, all payments made by the student to the school shall be refunded except:

o An amount equal to the enrollment fee, if any, specified in the contract or
expressly receipted for, not to exceed the sum of $10 or 10% of the total,
whichever is greater, specified cost of such course of instruction or series of
lessons; and

o The school’s per-lesson tuition charge for each lesson already taken by the
student. This charge shall be determined by dividing the total cost of such course
of instruction or series of lessons by the number of lessons included therein.

• If a refund is requested, the student automatically forfeits any package discounts, and
all services taken up to that moment will be charged as individual services. A 10%
service fee will be deducted from all refunds.

5. General Conduct and Engagement

5.1. Instructor Authorization:
• Instructors are not permitted to give private lessons or engage in any transactions
outside of those conducted directly through Madame Driving School Corp. All payments
and scheduling must be handled via the school.

5.2. Minor Students:
• If the student is under 18 years of age, the contract for services must be signed by the
student’s parent, guardian, or person in loco parentis who is 18 years of age or older.
By proceeding with Madame Driving School Corp.’s services, you acknowledge that you have
read, understood, and agree to these Terms and Conditions.
